Introduction An Electronic Health Record (EHR) is an electronic version of a patient medical history that is maintained by the provider over time (CMS.gov, 2012). They are patient-centered records making the information available instantly and secured. It can include all of the key administrative clinical data relevant to the patients care under a particular provider, including demographics, progress notes, problems, medications, vital signs, past medical history, immunization, laboratory data and radiology reports.
